Advance Health Care Directive & POLST: Making Your Wishes Known

Do you have an Advance Health Care Directive? None of us likes to think about circumstances that may render us unable to make medical decisions for ourselves, nor do we necessarily enjoy talking about end of life wishes with our loved ones.

Advance planning is the best way to ensure your wishes are honored and alleviates the worry of others about acting on your behalf. This presentation will provide up to date information on options for making your wishes known, including the POLST (Physician’s Order for Life-Sustaining Treatment) which is becoming the standard in health care and community settings.

Join us as Felicia Juntunen, Director of Care Management, with Elder Care Management, provides an understanding of the Advanced Health Care Directive and POLST, how to use them, and resources to help complete an Advanced Health Care Directive or update an existing one.
